Please download, fill out and print the correct order form for your order. We require a separate order form for each type of order being submitted. Movie and slideshow orders require 1 form per input. ie If sending slides and 3 VHS tapes, you need 1 form for the slides and 1 for each VHS tape (to allow you to specify a DVD template/text for each VHS tape.)
Some forms are 2 pages. Make sure both pages are filled out and submitted, including the signature at the end of each form. All ordered received with the correct order form will be returned at your expense.
